Music Application

Music students are required to complete this form in addition to the Student Application. Please mail an audition tape along with your completed applications.

Enclose the following with your application:

• A recent wallet-size photograph of yourself.
• A $20.00 non-refundable application fee must accompany your completed application. Checks or money orders should be made payable to Grace Church.

Personal Recommendation and Minister's Recommendation

One Personal Recommendation form and one Minister's Recommendation form are required.

Please complete the top portion of each form (name, address, phone number and signature) before giving it to the designated person. Attaching a stamped envelope addressed to Grace School of Ministry with your form will expedite your application process.

Notification by mail will be given to the applicant within 4-6 weeks upon receiving all completed application forms.
